PetPals Application

If you are interested in becoming a PetPals volunteer you must be 18 years old and be able to commit to 2-4 hours a month for at least 6 months. Please first review the requirements for your dog and then fill out the form below in addition to the MATCH-UP Volunteer Application.

Requirements for MATCH-UP PetPals Dogs
A. Be at least one year of age;
B. Be able to obey basic commands including “sit,” “stay” and “heel;”
C. Be on a flea and tick control program from March to November;
D. Have a physical exam at least 3 months before placement and a fecal exam at least 1 month before placement at a long-term care facility;
E. Have a licensed veterinarian fill out the Animal Health Screening Form (This will be sent to you when we receive your application.);
F. Attend the PetPals Dog Screening and Evaluation to be scheduled by MATCH-UP staff.
